We are happy to announce the publication of several papers based on the ICARE4EU project in the journal Health Policy. The papers will be published (open access) in a Special Issue on Integrated care and Multimorbidity early 2018. Now already available online are:

The ICARE4EU project partners proudly present five Policy Briefs that could help policymakers in European countries to adapt their health systems to better meet the needs of their citizens with multimorbidity.
Some 50 million Europeans live with multimorbidity and their numbers are likely to grow. These people have complex health problems and need ongoing care from multiple care professionals and organizations. Policymakers all over Europe are alarmed by the challenge this poses to their health systems and social services; many have put multimorbidity high on their policy agenda. The European Commission has funded the ICARE4EU project (DG SANTE's Health programme) to help policy makers to learn from new approaches to integrated care and to share the experiences of innovative practices in European countries.
These Policy Briefs are based on information from about a hundred integrated care initiatives identified by country experts all over Europe. They consider: how to improve the design of integrated care for people with multimorbidity; how to make new care models more applicable; and how to make implementation more effective.

- How to strengthen patient-centredness in caring for people with multimorbidity in Europe?
- How to strengthen financing mechanisms to promote care for people with multimorbidity in Europe?
- How can eHealth improve care for people with multimorbidity in Europe?

In 2016 the ICARE4EU project consortium collaborated with a team of experts from the UK, who wrote a Country Factsheet about policy and practices to care for people with multiple chronic conditions in the UK, and England and Scotland in particular. We are very happy to present this UK Factsheet to you, as it contains a wealth of information that could be very useful for all working on the development and implementation of integrated care for people with multimorbidity.

On 22 March, the EU funded project 'Innovating Care for people with multiple chronic conditions in Europe' (ICARE4EU) is organising a final symposium on
European perspectives on care for people with multiple chronic conditions in Brussels.
The symposium will be structured around plenary sessions and
panel discussions and provides a great opportunity to meet and discuss with policy-makers, researchers and professionals from all over Europe. Please view the

October 20, 2015 | Workshop at the EUPHA congress 15th October 2015 in Milan
During the 2015 EUHPA congress in Milan, the ICARE4EU partners gave a workshop
titled: "Innovative care practices for people with multimorbidity in Europe". The
workshop was chaired by François Schellevis and Ewout van Ginneken. Iris van der
Heide, Anneli Hujala, Sanne Snoeijs, Francesco Barbabella and Verena Struckmann
presented findings of the ICARE4EU project on behalf of all project partners.

first half of this year. We now present the Clinic for Multimoribidity and Polypharmacy in Silkeborg,
the Strategy for Chronic Care in Valencia
and Two projects of the Belgian PROTOCOL 3 Programme.

We are proud to present the ICARE4EU
State-of-the-Art report. This report provides an overview of innovative programmes and approaches in Europe
to address the challenges of care provision to people with multimorbidity. Information will be provided on
various characteristics of integrated care programmes for people with multimorbidity that have been identified
in 31 European countries.

of multimorbidity programmes which we selected from our database. The presented
cases were reviewed in-depth during site visits that occurred in the first half

The official launch
of the website will take place on June 1st 2013. With the launch
of this website information about the projects is publically
available and we are proud to present ourselves online. We hope
that the website will provide insight in the aim, tasks and
outcomes of the project and will enable those with questions or
remarks to contact us.
March 18, 2013 | Kick-off meeting
On March 18 the ICARE4EU
project was officially started with a kick-off organizations were
present. In a two day get together in Brussels, important issues
concerning the planning of the project and the tasks ahead were
discussed. It was also a good opportunity for the partners to get
to know each other.
